Health Pointe of NY’s approach is well-coordinated, comprehensive and holistic; assigning every I-SNP member a Nurse Practitioner (NP) that will be the Case Manager for the member. NPs work in conjunction with the SNF-based Interdisciplinary Care Team (ICT), and are the central link for care integration.
