In addition to your current care, Health Pointe of NewYork provides you with a Nurse Practitioner responsible for facilitating and coordinating your care needs. By choosing Health Pointe of New York, you can continue to see the facility physician as your Primary Care Physician.

You will be assigned to a Nurse Practitioner who will coordinate your care in collaboration with your Primary Care Physician and other members of the team in the nursing home you reside.


Your Nurse Practitioner will have time to answer questions and assist you and your family when making important health care decisions.

Your Nurse Practitioner will speak regularly with your family to provide updates, discuss any additional necessary care and explain changing needs as they occur.

Assist with Transition of Care

If you ever need to be hospitalized, your NursePractitioner will assist your physician in evaluating your Care Plan and monitoring your condition when you return to the nursing home.

Assistance with Advance Directives

Your Nurse Practitioner will help you and your family to understand Advance Directives and provide sufficient information so you can make an informed health care decision based on choice.

Your dedicated Nurse Practitioner will:

• Perform routine physical examinations in addition to your PCP visits

• Perform frequent monitoring during change in your health status

• Coordinate different aspects of your care

• Communicate regularly with you and your family

• Collaborate with your PCP and other members of the nursing home team

• Become a liaison and advocate for your care

Health Pointe of NY has in-depth knowledge of the special needs of frail, elderly and disabled individuals with complex and costly medical, behavioral, cognitive, functional and social issues, and is committed to the provision of quality, affordable, person-centered care for vulnerable populations. Health Pointe of NY’s approach is well-coordinated, comprehensive and holistic; assigning every I-SNP member a Nurse Practitioner (NP) that will be the Case Manager for the member. NPs work in conjunction with the SNF-based Interdisciplinary Care Team (ICT), and are the central link for care integration.
